The ADRC makes it easier to find the help you need—when you need it. From information and care options to short-term case management and benefits enrollment, we connect you with a full range of long-term care services and supports.

FAMILY CAREGIVER CENTER

The Family Caregiver Center provides resources, guidance, and relief for caregivers of adults age 60 and older, as well as for caregivers of individuals of any age living with dementia or related disorders. Our goal is to reduce caregiver stress, build resilience, and help families keep loved ones safe and supported at home.

PACE OF NORTHEAST INDIANA

PACE of Northeast Indiana brings together preventive, primary, acute, and long-term care services under one coordinated program. Our goal is simple: to help older adults get the care they need while continuing to live safely and comfortably in their own community.

GERIATRIC & DISABILITY CASE MANAGEMENT

This program takes a whole-person approach to care, promoting health and safety in community-based settings. Through personalized planning, ongoing coordination, and consistent quality of services, Case Managers ensure you have the right support to live independently for as long as possible.

Building safer communities for older adults. TRIAD is a nationwide initiative that brings together local law enforcement, social service agencies, and community partners to improve the safety and well-being of older adults.

TRIAD

TRIAD is a national effort headed by local law enforcement in partnership with local social service agencies to offer a variety of specialized programs on safety for older adults. 

ADVANCE CARE PLANNING

Advance Care Planning (ACP) is the process of making decisions about the medical care you would want in the future. It allows you to outline your wishes for treatment and designate a trusted health care representative to speak on your behalf if you’re unable to do so.
