AGING & DISABILITY CENTER (ADRC)

The ADRC makes navigating long-term care simple. From information and care options to short-term case management and benefits enrollment, we connect you with the services and supports you need to live well in the community—safely, confidently, and independently.

FAMILY CAREGIVER CENTER

The Family Caregiver Center provides community-based services and supports to reduce caregiver stress and prevent burnout. By offering resources, guidance, and respite, we help caregivers stay strong while ensuring their loved ones can remain safe and supported at home.

PACE OF NORTHEAST INDIANA

PACE of Northeast Indiana is an integrated healthcare program that combines medical, social, and long-term care services under one roof. With a dedicated team of professionals, PACE helps older adults stay healthy, supported, and independent—while continuing to live in the community they call home.

GERIATRIC & DISABILITY CASE MANAGEMENT

Our Geriatric & Disability Case Management program takes a comprehensive, community-based approach to support health and safety. Through individualized care planning, ongoing coordination, and consistent quality of services, we help older adults and people with disabilities maintain independence and thrive at home.

PROGRAMS FOR PRODUCTIVE AGING

Our Programs for Productive Aging help older adults stay active, connected, and engaged. From wellness activities and lifelong learning to volunteer opportunities and social events, these programs encourage healthier lifestyles and stronger community connections.
