Geriatric & Disability Case Management

Aging & In-Home Services’ (AIHS) Geriatric & Disability Case Management program is designed to promote health, safety, and independence in a community-based setting. Through personalized care, our Case Managers help delay nursing home placement and support individuals in living comfortably in their own homes for as long as possible.

With a focus on continuity and quality of care, our Case Managers work alongside you, your family, and your care providers to create and carry out a plan that meets your unique needs.

What Services Can I Receive?

Case Managers provide a comprehensive, hands-on approach to support your well-being. Services include:

  • Thorough Assessments – Understanding your needs, goals, and challenges.
  • Personalized Care Plans – Developing a customized plan just for you.
  • Service Coordination – Arranging the right supports and services to fit your situation.
  • Ongoing Monitoring – Checking in regularly to ensure your needs are being met.
  • Direct Support – Available anytime to answer questions or address concerns.

Eligibility & Costs

  • No Cost to Participate: Geriatric & Disability Case Management services are provided free of charge.
  • Program Requirements: Eligibility varies depending on the funding source. Programs may include:
    • Medicaid Waiver
    • Community and Home Options to Institutional Care for the Elderly and Disabled (CHOICE)
    • Social Services Block Grant (SSBG)
    • Title III Programs

Your Case Manager will walk you through which programs you qualify for and help you access them.
