What is Honoring Choices® Indiana?

Honoring Choices® Indiana is a statewide initiative dedicated to making Advance Care Planning a normal part of health care. The goal: to ensure that every Hoosier’s medical preferences are discussed, documented, and honored.

Through partnerships with hospitals, health systems, community groups, educators, and faith-based organizations, Honoring Choices® Indiana is building a culture where care decisions reflect personal values and goals, even in times of crisis or end-of-life care.

  • Part of a 13-state network sharing best practices and resources.
  • Coordinated locally by Aging & In-Home Services of Northeast Indiana in Fort Wayne, serving as the initial coordinator and fiscal agent.
  • Driving change through awareness, education, and policy improvements in advance care planning.
